Please fill out the sponsorship application form below. You will be prompted for payment by credit card or EFT on the next page. Applications are considered confirmed once payment is received.  

Registration for on-site representatives will open in June 2025. The applicant will receive instructions on how to complete representative registration from the CLHIA.

Sponsorship application information

Your on-site company representative should complete this application. If you don't know who your on-site sponsor representative is right now, apply as yourself for now. We can replace you with your on-site representative's information later, or you can add them as a representative when general registration opens in June 2025. Instructions will be emailed to you at that time.

The deadline to register your sponsor representative is Wednesday, July 30, 2025.
